We offer innovative programs and health services to reinforce healthy behaviors and lifestyles to keep our enrollees healthy.

CareFirst CHPDC has a Health and Wellness Center. Our centers provides a sense of community and accessibility for our enrollees.
We offer face to face enrollee services, care coordination, case management, health screenings, health education, healthy cooking demonstrations, physical fitness programs and enrollee resources.

Enrollee Resources

Onsite Case Management
Health Management Support, Diabetes Education, One-on-One Case Management, Nutritional Classes, Development of Care Coordination Plan, Discharge Planning, Transportation.

Enrollee Wellness Center
Assist enrollees with Care Coordination, Recertification, New Enrollee Orientations, Enrolling in Health Programs and Enrolling in Case Management.

Enrollee Services
Address enrollee's concerns or complaints, Update enrollee's PCP, Arrange transportation, Issue temporary ID cards, Assist enrollees with enrolling case management.

Resource Libraries
Onsite and online library, offering extensive information about Healthy Behavior, Pediatric Care, Diabetes, Maternal Health and Pediatric Asthma. Our collection of books includes a variety of topics including disease-specific information, prevention strategies, and treatment alternatives, coping with chronic illness, in addition to Health Education movies, CDs and audio books.
