CareFirst CHPDC health programs for all enrollees
At CareFirst CHPDC, we provide access to a variety of health care programs to help assist our enrollees and their families in living a healthy lifestyle. These programs can be found below.
Adult Diabetes Disease Management Program
CareFirst CHPDC has a Diabetes Disease Management Program. This program is FREE of charge, if you have a diagnosis of Diabetes. Disease Management is an organized effort aimed at achieving desired health outcomes in populations with prevalent, often chronic diseases. If you would like to be part of this program, your Disease Manager may provide some or all of the following services:
Download Brochure- Support regular contact with your primary care physician.
- Provider one to one discussion either face-to-face or over the phone to discuss your health conditions, medications, labs, prevention measures or other needs.
- Assist you or your caregiver in the management of diabetes and other health conditions to promote the best outcome to improve your health.
- Review and help you understand your Diabetes Action Plan to help reduce health risk in individuals with Diabetes.
This Disease Management program will help you or your caregiver to utilize all community resources that CareFirst CHPDC offers it enrollees, such as Diabetes Education Classes, Nutritional classes either one to one/and or in a group setting, frequent monitoring of blood glucose, and glucometer teaching and much more.

Pediatric Asthma Disease Management Program
CareFirst CHPDC has a Pediatric Asthma Disease Management Program. This program is FREE of charge, for enrollees ages 2 to 20. If you would like to be part of this program, your Disease Manager may provide some or all of the following services:

- Educational Materials
- An Asthma Action Plan
- PCP Appointment Assistance
- Assessment and education regarding Asthma medications and equipment
- Home visits to detect “allergens” in the home that make Asthma symptoms worse
- Educational Seminars at the CareFirst CHPDC Health and Wellness Outreach Center
If needed, CareFirst CHPDC may provide several services to you through Breathe DC, including:
- Pest control
- HEPA vacuums
- Allergen tests
- Mattress and pillow case covers
- Furnace filters
- Other items that will help reduce the allergens in your home
- Smoking cessation counseling

HealthCheck (EPSDT) Program
If your child is in the DC Healthy Families (Medicaid) program, your child will be in the HealthCheck Program, also called Early and Periodic Screening, Diagnosis, and Treatment (EPSDT). This program starts right after your child is born and lasts until your child turns 21. The HealthCheck Program gives your child a number of important check-ups. Your child's PCP will provide or help your child get these services.

Strong Start/ IDEA Program
IDEA stands for the Individuals with Disabilities Education Act. IDEA is a federal law. The IDEA program provides special services for your child with developmental delays, disabilities, or special needs. To get an IDEA evaluation, call your PCP. If your child needs IDEA services, your PCP will refer your child to the DC Government's Early Intervention Program. CareFirst CHPDC has case managers who can tell you more about IDEA and the other services your child can get.
