Alliance Enrollees
Your ID Card

Once you have signed up with CareFirst CHPDC and have chosen your primary care provider (PCP), enrollee identification (ID) card will be sent in the mail. When you receive you card, you will want to look it over and confirm that all information included on the card is correct. This card should include your full name, your date of birth, your enrollee ID number, and your primary care provider’s information.
If any of the information is incorrect please contact us (202) 821-1100 or (855) 326-4831, and a new, corrected card will be issued.
Your enrollee ID card will identify you as a enrollee of CareFirst CHPDC and will be used every time you visit your doctors, a hospital, drug stores, etc. It is important to carry this ID card with you at all times as you are required to show the card before receiving medical or pharmaceutical services of any kind.
